I understand that the information I provide is for the purpose of receiving massage and wellness services. I consent to the collection and storage of this information by Massage Yogini, LLC for client care and recordkeeping.

I acknowledge that reasonable administrative and technical measures are taken to protect my personal information, and I consent to communication via email or electronic forms.

I consent to receiving appointment-related communication via email, including intake forms, scheduling, and follow-up care.

Massage Yogini, LLC collects personal information such as name, contact details, and health information solely for the purpose of providing massage and wellness services. This information is kept confidential and is not shared with third parties except as is required by law.

By using this website and submitting forms, you consent to the collection and use of your information as described.


Photo Identification:

A government issued photo ID is required and is to be submitted with the New Client Intake form for ALL first time clients for safety reasons. Massage Yogini, LLC reserves the right to refuse services at any point.


Minors:

Massage Yogini, LLC provides services exclusively to clients 18 years of age and older. Clients must be at least 18 years old at the time of their appointment. No exceptions will be made. Individuals under the age of 18 are not permitted to receive massage services, even with parental or guardian consent, and may not accompany clients into the treatment space.


Massage Yogini, LLC reserves the right to refuse service if age requirements cannot be verified or are not met.


Scope of Practice:

Massage therapy services provided by Massage Yogini, LLC are intended for relaxation, stress reduction, and relief of muscular tension. Massage therapy is a non-invasive therapeutic approach to wellness and is not a substitute for medical care.

As a licensed massage therapist, I practice within the scope defined by the Indiana Professional Licensing Agency and applicable state laws and regulations. Clients are encouraged to consult a qualified healthcare provider for any medical concerns, diagnosis, medications, or treatments for specific medical conditions.

  1. Services may be refused at the therapist's discretion.

  2. Medical clearance my be required for certain conditions.

  3. Clients are responsible for disclosing all relevant health information.


By receiving services, clients acknowledge and agree to this scope of practice.


Intake Form Completion Policy:

All clients are required to complete their intake forms prior to their scheduled appointment time.

Intake forms are sent electronically and should be submitted before arrival to ensure a smooth and uninterrupted session.

If forms are not completed in advance, clients must arrive early to complete them before their appointment begins. Please note that appointment times will not be extended to accommodate incomplete forms. Any time spent completing intake paperwork at the time of our appointment will be deducted from your scheduled session.

Clients who arrive at their scheduled appointment time without completed forms may experience a shortened session, as all appointments will end at their originally scheduled time.

Payment Policy:

All services must be paid in full at the time of booking through the Massage Yogini, LLC website. Accepted payment methods are those available through the online booking system.

Appointments will not be confirmed without completed payment.

Massage Yogini, LLC operates as a prepaid service-based business.


By booking an appointment, you agree to the following:


  • Full Payment Required: Payment is due in full at the time of scheduling to reserve your appointment,

  • No Cash or In-person Payments: All transactions are completed securely online. No payments are accepted at the time of service.

  • Non-Transferable Services: Appointments are non-transferable and may not be reassigned to another individual without prior approval.

  • Late Arrivals: Sessions will end at the originally scheduled time regardless of late arrival.

  • Refunds: Payments are non-refundable. In the event of an approved cancellation or reschedule within policy guidelines, a credit may be issued for future use.


Massage Yogini, LLC reserves the right to refuse service if payment has not been completed or if policies are not upheld.


Right to Refuse Service:

Massage Yogini, LLC reserves the right to refuse or stop a session at any time for:

  1. Safety Reasons

  2. Health Contraindications

  3. Inappropriate Behavior

Communication and Consent:

Open and respectful communication is encouraged regarding pressure, comfort, and boundaries. Clients may request adjustments at any time. the therapist will also check in as needed to ensure client comfort and safety.


Cancellation Policy:

Massage Yogini, LLC requires 24 hours notice to cancel an appointment.

Cancellations made at least 24 hours prior to the scheduled appointment time may be eligible for a credit toward a future session. No refunds will be issued.

Cancellations made less than 24 hours before the scheduled appointment time will be considered a late cancellation and will result in forfeiture of the session and full payment.

Clients who do not provide notice and fail to arrive for their appointment will be considered a no-show and are subject to the No-Show Policy.

All cancelations must be made through the online booking system or by direct communication to be valid.


Rescheduling Policy:

Massage Yogini, LLC requires a minimum of 12 hours notice to reschedule an appointment.

Clients may reschedule their appointment without penalty if notice is given at least 12 hours prior to the scheduled session time. Rescheduled appointments will retain their prepaid value as a credit toward a future session.

All rescheduling requests must be made through the online booking system or by direct communication and must be confirmed to be valid.


Pregnancy Policy:

Massage Yogini does not provide Prenatal Massage services. Clients who are pregnant must disclose their status, and services may be declined at the therapist's discretion for safety reasons.


Illness and injury:

Full disclosure of medical conditions is necessary for the safety of the client and practitioner. Certain medical conditions may have contraindications to treatment. After evaluating the client's health, the therapist may become aware of circumstances that will not allow massage.

Massage is not appropriate care for infectious or contagious illness. Please cancel or reschedule your appointment as soon as you are aware of an infectious or contagious condition. This includes fevers within 24 hours of session.

Many injuries are contraindicated to massage and require a Physicians Release Form before massage can be done. If you have been injured please reach out in time for us to decide the necessary steps that need to be taken prior to massage. Please cancel or reschedule your appointment as soon as you are aware of any injury that may prevent massage. If cancelation is within the 24 hour period, the cancellation fee may be waived. If rescheduling is done within the 12 hour period, the cancellation fee may be waived. Ultimately, it is decided by the practitioner. If there are issues please reach out as soon as possible.


Draping and Professional Boundaries Policy:

Clients will be properly draped at all times and in accordance with professional standards and state regulations.

  1. Only the area being worked on will be exposed.

  2. Genital and breast areas will not be massaged under any circumstances.


Client Conduct, Harassment, and Inappropriate Behavior:

There is absolutely no tolerance for harassment of any kind. Verbal, physical, or sexual harassment will be addressed immediately and appropriately. Any behavior that is suggestive, inappropriate or sexual in nature will not be tolerated. This includes comments, requests, gestures, or actions that violate professional boundaries. If such behavior occurs, the session will be terminated immediately. If boundaries are crossed you will be asked to leave at once. Full payment will be required for the scheduled time, even if you have not received services.


Zero Tolerance Policy:

Any form of sexual misconduct or harassment may result in permanent refusal of services and, if necessary, reporting to appropriate authorities.

Massage Yogini, LLC operates in accordance with the standards set by the Indiana Professional Licensing Agency.


Late Arrival and No Show Policy:

Massage Yogini, LLC operates on a prepaid, appointment-based schedule, Your reserved time is held exclusively for you.


  • No-Shows: Clients who fail to arrive for their scheduled appointment without notice will be considered a no-show. No-shows result in the full forfeiture of the session and payment. No refunds or credits will be issued.

  • Late Arrivals: Clients arriving late will receive the remaining portion of their scheduled session time. Sessions will end at the originally scheduled time to respect other clients and the practitioner's schedule.

    If a client arrives more than 20 minutes late, the appointment may be canceled at the therapist's discretion and will be treated as a no-show.

  • Repeat Occurrences: Multiple no-shows or habitual late arrivals may result in refusal of future booking privileges.


Massage Yogini, LLC reserves the right to uphold all policies to protect time, energy, and availability for all clients.
